At the start of the season, few pundits would have been surprised to learn one of these sides would be pushing for a Champions League spot while the other would be languishing in mid-table at this point in the season.
However, it is Tottenham who are currently looking up enviously at West Ham, who occupy fifth spot in the Premier League table and could leapfrog Chelsea with a win.
David Moyes has revitalised the club in his second spell, making a number of shrewd acquisitions in the transfer market and injecting newfound optimism and confidence into several of the more senior players.
Conversely, Jose Mourinho's men have lost two on the bounce on the road and have lost four of their last five to place an even bigger emphasis on the derby.
And former Chelsea striker Cascarino believes that the Hammers can exploit the fragile defence of the visitors on Sunday afternoon with Michail Antonio restored to the starting line up.
"West Ham are a vastly improved team, as we have talked about many times," Cascarino told Natalie Sawyer.
"They don't give you anywhere near as much, they are a much more effective team, they play with a lot more purpose than they did last season and David Moyes seems to trust a lot more players.
"He has got the basis of the type of side that he would, so it's a real challenge for them [Spurs].
"They are a different proposition from last year West Ham and, going into this game, defensively I still think Tottenham are weak.
"That could ultimately be the deciding factor in the game."
While Trevor Sinclair have singled out World Cup winner Hugo Lloris for his somewhat erratic performances in recent weeks, Cascarino highlighted £42million signing Davinson Sanchez as a potential weakness for Spurs.
The former Ajax centre-back has struggled for consistency since his move in 2017 and Cascarino believes Jose Mourinho is unable to fully trust the Colombia international.
"I think it has been really difficult for him," he added. "He has found himself in a top club, with a not very good back four or five at the moment.
"Everyone is making individual errors, he has made a few himself, and I think Jose has found it very difficult to find a defensive backline that he really trusts.
"Davinson Sanchez has been in and out of that team like many others in that Spurs backline."
