Romelu Lukaku looked to have scored his second and the winner with just a few minutes remaining but it was ruled offside by a fraction.
Hernandez's strike in the 90th minute sealed the deal and secured a comeback that echoed another dramatic match the night before.
Ferran Torres' brace in the first half also looked to give Spain a dominant victory over Italy, helped on by Leonardo Bonucci's red card.
Somehow the Azzurri fought back with a goal from Lorenzo Pellegrini and made it a nail-biting affair to the final whistle where Spain ultimately held on and toppled Italy's unbeaten run.
Now the two sides will be at it again this weekend.

The Nations League Final will take place on Sunday, October 10 with a kick-off time of 7:45pm.
