Charlie Morgan was on the receiving end from the former Chelsea winger during the second leg of a League Cup semi-final in 2013.
Hazard was sent off for kicking Morgan as he tried to retrieve the ball in his side's loss to eventual winners Swansea.
The ex-ball boy went on to found Au Vodka with his best friend Jackson Quinn just two years later, with the business now worth £150m.
Au Vodka's growth has seen Morgan, who is now managing director, unveil the company's new headquarters in Swansea.
Showing off the site on LinkedIn, he shared pictures of the black and gold interior to the building.

The building also features a room stocked with various flavours of Au Vodka in their striking gold bottles.
Morgan, Jackson and DJ Charlie Sloth also posed outside the new HQ whilst flanked by gold cars and vans displaying the brand's logo.
Sloth is an investor in Au Vodka and has actively promoted the company since becoming involved.
Alongside the snaps, Morgan wrote: "Introducing… the new Au Vodka HQ.

"We spent four years in our first office which was only 250 sq ft and only just fit myself, Jackson Quinn & Jordan Major (who was just 16 and packing orders!).
"We spent five years in an industrial unit in Swansea, where we grew the business exponentially and the team to 96 people, which we added a new mezzanine floor to house more people four times.
"Today we've moved into our new home - A fully gold HQ which really represents Au and what we stand for."
As well as Sloth's involvement in Au Vodka's marketing, the brand has also been promoted by the likes of Floyd Mayweather and Ronaldinho.
Jake Paul even had their bottle temporarily tattooed on him to promote the business.
Meanwhile, Au Vodka recently collaborated with Luke Littler to celebrate the world darts champion's 18th birthday.
Littler was presented with a personalised bottle of their vodka as well as a giant golden dart.
His nickname 'The Nuke' was also written on the giant dart, which was presented by Au Vodka sales director Tom Smith.
