In a thrilling match at the London Stadium, Jesse Lingard returned to haunt his former on-loan club with a stunning late goal, but it was his keeper who stole the headlines with a spot-kick save five minutes into injury time - and it was a long time coming.
It was a remarkable occasion for both the shot stopper and the taker, who have had consistent results from the penalty spot, with very different fortunes.
On 40 occasions prior to Sunday's Premier League clash, De Gea failed to save each and every spot kick that came his way.
For a keeper of his stature it would have been a bitter pill to swallow to see the ball buried into the net every single time. It would have been even more painful during his club's loss in last season's Europa League final, where the match was decided on penalties.
All ten of this teammates converted in the shoot-out while he failed to save every single attempt from opponents Villarreal, and to make matter worse he then missed the decisive kick in sudden death.
But while the Spaniard has now earned redemption, West Ham club captain Mark Noble will have to deal with the criticism, as will his manager David Moyes for his surprise decision to bring him off the bench to take the crucial spot kick cold.
Noble had not missed from the spot since December 2016, having since successfully converted ten penalty kicks.
The midfielder has boasted a stunning penalty record throughout his one-club career for the Hammers, scoring 35 goals from the spot and missing just four ahead of Sunday's clash against United.
It's clear to see why Moyes made his decision, but to introduce the 34-year-old off the bench rather than pick one of the players already on the pitch to take such an important kick has led to severe criticism.
Man United legend Roy Keane and Liverpool great Graeme Souness were both on punditry duty for Sky Sports, and they didn't pull any punches as they had their say.
"People will say he's taken penalties before, he's an experienced player… I don't care," said Keane in typical no-nonsense fashion.
"Strange decision. He can't have warmed up properly because it was so late in the game. I didn't fancy him, I thought De Gea would save it.
"There must be players on the pitch who are up speed with the game, [Declan] Rice, [Aaron] Creswell, whoever is on the pitch, slot the ball away.
"If he [Noble] scores again it's all justified, Gareth Southgate was criticised but to bring someone on who hasn't had many minutes…
"Sometimes as a manager you try be too clever, you can say he's taken penalties before and he's experienced, but I don't care."
"It's a poor decision," added Souness.
"It's the 94th minute, he's not been warmed up to come on so he's stone cold.
"Stone cold, doesn't matter if he scores every penalty he takes in training and every penalty before today, you don't bring someone on whose stone cold and been sitting watching the game for 90 minutes to take a penalty.
"It's wrong."
