The two sides are unable to finish in the top two in Group D, with Manchester City and Barcelona set to advance to the quarter-finals.
Match preview
Having lost all five games in the main, St Polten are one of three teams yet to pick up a point in this season's group stage.
They fell to a 2-0 defeat in their opening group game against Hammarby, before they gave up a 2-1 lead to lose 3-2 to Manchester City in their second group match.
St Polten then experienced two difficult outings against reigning European champions Barcelona, losing 7-0 in the away match and 4-1 on home soil.
Unfortunately for the Austrian champions, another defeat followed in last week's trip to England, as they fell to a 2-0 loss against Manchester City following second-half efforts from Lily Murphy and Kerstin Casparij.
While they cannot reach the quarter-finals, St Polten still have the chance to move off the bottom of the group standings, but they will have to beat Hammarby by at least two goals to achieve a third-place finish.
Hammarby are preparing for the final game of a successful European campaign despite failing to make the quarter-final stage.
The Swedish side's real achievement was reaching the main draw in their maiden UWCL campaign, overturning a 2-1 first-leg deficit in their qualifying clash with Benfica to seal their place in the group phase with a 3-2 aggregate victory.
Unfortunately for Hammarby, they have been unable to make a significant impression in a tough group despite being the main draw with a 2-0 win over St Polten.
They followed that result with four consecutive defeats in their home and away fixtures against Barcelona and Man City, conceding 16 goals and scoring just once in that period.
Hammarby will now exit the competition following Wednesday's fixture, but they will still be keen to sign off with their first away win in the UWCL main draw, as they look to hold on to third place in Group D.
