In the final, Forest beat third-placed Huddersfield, who had seen off Luton Town in the playoff semi-finals.
The playoff final was decided by a Levi Colwill own goal, which saw the two-time European champions promoted to the Premier League. They join Bournemouth and Fulham in going up from the second tier.
When Cooper arrived Forest were second bottom in the Championship, ahead only of Derby County, who had been hit with a 21-point deduction.
