If the Citizens beat Aston Villa on Sunday, they will be crowned champions for the fourth time in five seasons, no matter what Liverpool manage.
But where do City rank on the all-time list?
Here talkSPORT.com takes a look ahead of a dramatic final day of the 2021/22 campaign.
Manchester United - 13
Manchester United are the most successful team in Premier League history, but they now haven't won the title since the 2012/13 season - Sir Alex Ferguson's last year.
Under the fiery Scot, the Red Devils dominated the 90s, winning six of the first eight titles.
That dominance continued into the 2000s, as United continually and successfully evolved their squad.
However, since the Scotsman retired as manager, the Red Devils haven't won another title and look as far away as they have been this year.
Manchester City - 5
Manchester United's inner-city rivals City have become the new force in English football.
They won their first league title during the 2011/12 campaign thanks to that iconic goal from Sergio Aguero.
They were victorious again during the 2013/14 season, although many feel that Liverpool handed them the title that year.
But since Guardiola arrived, his side have blown away all of the competition, and they could win another title on Sunday ahead of Liverpool.
Chelsea - 5
Chelsea's rise to the top came under the ownership of Roman Abramovich, which has now come to an end.
The Blues won back-to-back league titles under Jose Mourinho and became just the fourth team to win the Premier League during 2004/05.
They had further success under Carlo Ancelotti and Antonio Conte.
However, the Blues haven't won the league since the 2016/17 campaign - Conte's first as Chelsea boss.
Arsenal - 3
The Gunners became Man United's main competitors for the league during the late 90s and early 2000s.
Arsene Wenger and Sir Alex both built brilliant sides, but despite this, Arsenal have only won three Premier League trophies.
Still, the Gunners boast one of the greatest teams the league has ever seen, as the 2003/04 squad assembled by Wenger went through the entire 38-game campaign unbeaten.
No one has managed that since, though it was the last time Arsenal lifted the title.
Liverpool - 1
Despite being one of the world's biggest clubs, Liverpool haven't actually had that much success in the Premier League.
They have only lifted the trophy once during the disrupted 2019/20 campaign.
Jurgen Klopp has done some amazing things at Anfield and that first title win will mean he will forever go down in Liverpool folklore.
He could add a second on Sunday, but the Reds need former hero Steven Gerrard and his Aston Villa backroom staff to do them a favour against Manchester City.
Leicester City - 1
The 2015/16 campaign will always be remembered as one of the best as 5000/1 outsiders Leicester were crowned champions.
The Foxes spent very little on assembling this side, but incredible displays from the likes of Riyad Mahrez, Jamie Vardy and N'Golo Kante helped them win the league.
Tottenham and Arsenal were the other contenders, but the Foxes just kept winning.
They had only lost three games by the end of the year and deservedly lifted the trophy.
It is unlikely we will ever see anything like that again.
Blackburn Rovers - 1
Blackburn upset the apple cart during the 1994/95 season by beating Man United to the title.
Their strike partnership of Chris Sutton and Alan Shearer proved to be absolutely lethal.
Unfortunately, the side was then dissembled in the summer, as many moved on to pastures new.
Blackburn aren't even in the Premier League now and haven't been since the 2011/12 campaign, but they will never forget this year.
